Informed consent is required prior to any surgical procedure. This form outlines key details regarding anesthesia, potential surgical risks, and what to expect before, during, and after the procedure. Please read the information carefully and complete the required sections.
What You Need to Know
Routt Home Vet is committed to providing safe, high-quality care during all minor surgical procedures. Although rare, anesthesia and surgery carry inherent risks, which may include (but are not limited to) allergic reactions, respiratory issues, or complications related to pre-existing conditions.
Dr. Hillemeyer takes every precaution to minimize risk and will monitor your pet closely throughout the procedure and recovery period.
What’s Included with Surgery
Your pet’s surgery may include:
- Pre-surgical physical exam
- Anesthesia and monitoring
- Pain management before and after the procedure
- Local or injectable antibiotics (as needed)
- Post-operative care instructions
Some procedures may require follow-up visits or additional treatment recommendations, which will be discussed with you in advance.
